Job Details:

Provides assessments to determine patients rehabilitation needs and psychosocial status

Assesses each patients environment and recommends adaptive equipment and/or home modifications as needed

Establishes an effective physical therapy treatment plan in partnership with the patient, their family/caregiver, the patients physician and other members of the health care team; modifies the treatment plan as needed

Provides physical therapy services in accordance with the treatment plan, agency standards, and regulatory guidelines

Helps patient achieve independence at home through activities of daily living training, functional activity training, visual/perceptual training, psychosocial interventions, and heat/cold application

Provides patient and/or caregiver education regarding the injury/disease process, functional limitations and the purpose of treatment interventions. Trains patients and/or caregivers in aspects of care that will be managed independently after discharge

Plans adequately for discharge including referrals to appropriate community resources

Communicates regularly with the patients physician, home health care team, and other key members of the care team

Job Requirements

Bachelors, Masters or doctoral degree in physical therapy Currently licensed as a physical therapist in the State of Maryland, Virginia or the District of Columbia One-year clinical experience preferred Knowledge of home equipment, home care reimbursement, regulations governing Physical Therapy Assistants (PTAs not applicable in DC) Good verbal and written communication Good clinical skills Able to transfer, position patients and lift up to 25 lbs; able to perform physical assessments, walk and climb stairs Must have reliable transportation to all work sites CPR certification preferred###HR Use Only: *CB*
