Clinical ConsultantMake a Positive DifferenceClinical consultants at MedStar Health VNA make a positive difference in peoples lives by helping doctors, case managers, nurses and other referral sources find appropriate homecare services for their patients. Clinical consultants use their expertise to transition patients from acute and sub-acute care facilities into the comfort of their own homes. To create the best experiences for patients and referral sources, clinical consultants work to create trusting relationships with team members, internal partners and external customers. The ideal person for this position is a self-directed team player with clinical expertise, excellent customer service skills and a patient-first mindset.Job Details:Team Work: Collaborates with the homecare coordination team in the daily routines of referral development and monitors daily census; keeps detailed daily logs/statistics; prioritizes and organizes workload. Consults with and supports referral sources and MedStar Health VNA team members on complex cases.Customer Service: Builds and maintains effective working relationships with referral sources, key stakeholders and other departments; participates in meetings and on committees that support the department and MedStar Health VNAs marketing and outreach efforts.Productivity/Budget: Achieves or exceeds budgeted admissions through effective referral development.Expert Resource: Serves as expert resource person on home care issues and practice; provides education regarding home health and infusion services available.Technology/Workflow: Uses technology to maximize efficiency, accuracy and communication of daily referrals; proactively develops workflow practices that support MedStar Health VNA needs.Requirements: Registered nurse, physical therapy or medical social work license. Current professional license in Maryland, the District of Columbia or Virginia, or any combination as required based on the workplace site(s). Ability to read and write English, perform arithmetic calculations and communicate with patients, their relatives, physicians and a variety of MedStar Health personnel at a level normally achieved by obtaining a Bachelors Degree. Demonstrates competency in the delivery of care and applies the knowledge to meet the specific needs of the patient population. Minimum two years acute care or home care experience or equivalent. Self-directed team player with strong interpersonal and communication skills. Computer literate. Valid drivers license with access to transportation.
